Titel A Novel Virtual Planning of Bone, Soft tissue and Vessels in Fibula Osteocutaneous Free Flap with the Uppsala Haptics-Assisted Surgery Planning (UHASP) System
Sammanfattning Virtual surgical planning has proven useful for reconstructing complex congenital and acquired defects in the head and neck area. Benefits include increased accuracy resulting in improved function and aesthetics, and decreased intraoperative time yielding reduced morbidity and cost savings. Available systems need long lead-time to surgery, often requiring a technician to assist the surgeon, and implant and plate manufacturing to be outsourced. Furthermore, such systems simulate only bone reconstruction and do not include soft tissue for fibula osteocutaneous free flaps (FOFF). The Uppsala Haptics-Assisted Surgery Planning (UHASP) system comprises software that relies on stereo graphics and force feedback from a haptic device to guide surgery planning. After minimal training, a cranio-maxillofacial surgeon can in one hour, without a technician, develop a pre-operative, patient-specific plan for restoring the skeletal anatomy of the face and neck region. The system supports: (1) repositioning of skeletal bone fragments in complex facial trauma cases, (2) use of FOFF in head and neck reconstruction, and (3) design of titanium scaffolds implant for bone substitutes. Estimates in the literature suggest that a detailed surgery plan may shorten surgery by 25%. UHASP reduces the surgery time further both as a result of the insight the surgeon gains during the planning process and with the addition of soft tissue modeling. UHASPís shorter planning process, surgery, and postoperative recovery lead to considerable cost savings as does in-house production of patient-specific cutting guides and fixation plates. UHASP is applicable to general orthopedic and neuro-surgery and also as a teaching tool.
